7/5/2023 EnchantaVaughn Approved (AM) Upon arrival, client was observed arriving to school in a jovial mood. Client waved to teachers and peers as he passed them in the hallway. Client picked up breakfast and headed to class before the bell rung. Client was observed in a positive mood. Client was cooperative during morning annoucements. Client was observed joining his peers in the morning group activity. Client seemed to be starting off the week on a good note. Client gave good eye contact and was respectful to his teacher at this time. Client displayed good listening skills. As the client prepared for psycho educational group he was witnessed lining up without pushing or shoving and waiting for his turn to go. Client transitioned smoothly into his therapy session. Client was observed paying attention to the lesson being taught and behaving appropriately. (PM) Client was observed transitioning to the cafeteria for lunch time. After lunch, client went into the gym to engage in extracurricular activities such as basketball, tag, and team games. Client was observed treating others fairly and working together. The client was seen in group setting adhering to rules and regulations. Client was monitored giving his peers personal space and respecting their boundaries. Client was responsible for engaging in age appropriate activities and socializing with the other mental health clients. Client seemed to be making a lot of friends and opening up more to his group as the day went by. Client did an excellent job transitioning from setting to setting without incident. Client maintains appropriate conduct around his peers. Client is supported with making good decisions and following rules. Client went into the library with his group to engage in a reading activity. Client was seen brain storming his ideas, responding in a positive way to ice breakers, and contributing to group discussions. Client implemented relaxation techniques when he felt overwhelmed and tired. Client tries to avoid certain activities that he’s not interested in. Client demonstrated he can sit down and listen if he wants too. Client held appropriate conversations with peers. Client participated in each assignment provided cooperating with the instructor and being a team player. Client did not require redirection at this time. Client was praised for his excellent behaviors and following directions throughout the day. (EOD) Client was observed in his social area, interacting well with peers. Client was compliant to authority requests. Client exhibits low hyperactive and impulsive behaviors. Client is learning to adapt to the new schedule and switching classes by returning to class on time. Client had a great start to his week and is expected to return tomorrow with the same attitude.
